    Job Summary
    The Clinical Care Coordinator is a Licensed Practical Nurse (LPN) or Registered Nurse (RN) registered in the State of Kansas responsible for facilitating and providing Chronic Care Management (CCM) services to meet the needs of high-risk patients, while promoting quality and cost-effective outcomes.
    Responsibilities and Duties
    • Serve as the contact point, advocate, and informational resource for patients, care team, family/caregiver(s), payers, and community resources
    • Identify care needs for scheduled patients with high-risk conditions and communicates daily to the care team
    • Assist with the identification of high-risk patients and encourage enrollment in the Chronic Care Management program
    • Provide ongoing contact with patients, via telephone and in person
    • Work with high-risk patients to plan and monitor care; develop a care plan with the patient, family/caregiver(s) and providers
    • Educate patient and family/caregiver(s) about relevant community resources
    • Comply with Policies and Procedures of the Clinic
    • Other duties and responsibilities as assigned
    Qualifications and Skills
    • LPN or RN with a current license in the State of Kansas
    • Practical knowledge of medical office procedures
    • Evidence of essential communication, education, and counseling skills
    • Ability to work productively, either independently or with other individuals
    • Highly organized with the ability to keep accurate notes and records
    • Capability to effectively communicate with patients to implement successful care plans and strategies
    • Demonstrate a positive attitude and respectful, professional customer service
    • Acknowledge patient’s rights on confidentiality issues and follow HIPAA guidelines and regulations
